Our online referral form allows community partners to connect households in need with our services quickly and easily. Families referred to Mustard Seed are often transitioning out of homelessness, fleeing domestic violence, rebuilding after crisis, or working to establish safe and stable housing.
Partner Referrals Only
To ensure families receive coordinated support, Mustard Seed accepts referrals only from partner organizations and case managers.
We are unable to accept self-referrals through this form.
By submitting a referral, you are confirming that you are a staff member, volunteer, or case manager working with the organization listed in the referral form and that you are referring a household you are actively assisting.
If an individual contacts you seeking assistance, please encourage them to connect with a local social service agency or housing provider who can submit a referral on their behalf.
